GENERAL PURPOSE OF JOB
The Health Center Lead, RN is a patient-facing clinician responsible for delivering high-quality care while supporting the day-to-day operations of the health and wellness center. Approximately 90% of this role is dedicated to direct patient care.
In addition to clinical responsibilities, this role provides on-site coordination and operational support to promote efficient workflows, team effectiveness, and a positive patient experience. The Health Center Lead partners closely with operations leadership and serves as a resource for staff, helping address routine operational needs and escalate issues as appropriate.
KEY RESPONSIBILITIES
- Provides nursing care in accordance with the Nursing Code of Ethics and established clinical standards, policies, and procedures.
- Assesses, plans, implements, and evaluates patient care based on age-specific physical, psychosocial, and cultural needs.
- Collects and documents patient information, including chief complaint, medical history, vital signs, and other relevant clinical data.
- Administers medications, immunizations, and treatments safely and accurately in accordance with provider orders.
- Performs laboratory and diagnostic testing, including specimen collection, processing, and timely reporting of results, including immediate escalation of critical values.
- Assists providers with examinations and procedures, ensuring appropriate preparation of equipment and exam rooms.
- Provides patient education focused on wellness, prevention, and self-care based on individual needs.
- Supports care coordination and case management by serving as a patient advocate and resource for patients and families.
- Maintains accurate, timely, and compliant documentation in the electronic medical record.
- Adheres to infection control, safety, and OSHA standards, including use of universal precautions.
- Maintains patient confidentiality and ensures compliance with HIPAA and organizational policies.
- Supports day-to-day health center operations, including workflow coordination and resolution of routine operational issues.
- Serves as an on-site resource to staff, providing guidance and support to ensure efficient and effective clinic operations.
- Assists with staff scheduling, including maintaining schedules, communicating changes, and coordinating coverage as needed.
- Monitors and supports completion of operational tasks such as work queues, electronic logs, and supply management.
- Collaborates with operations leadership to identify opportunities for process improvement and enhance clinic performance.
- Supports onboarding, training, and ongoing development of staff.
- Provides input into staff performance through feedback and participation in performance review processes.
- Supports and participates in corrective action processes in partnership with Operations Leadership, as applicable.
- Promotes adherence to organizational policies, procedures, and quality standards.
- Acts as a liaison between staff, patients, and leadership to support communication and issue resolution.
- Ensures a high level of patient experience by supporting service excellence and addressing patient concerns.
- Assists with incident reporting and follow-up as needed.
- Maintains EHR Super User knowledge and provides support to team members as appropriate.
- Monitors the clinic environment and escalates issues as needed, offering solutions to support continuous improvement.
- Participates in quality improvement initiatives, continuing education, and organizational programs.
- Performs other duties as assigned.
JOB REQUIREMENTS
Education:
- Graduate from an accredited nursing program, BSN preferred
Experience:
- Must have at least three (3) years of experience working in a primary care setting and familiarity with an electronic medical records system
- One (1) year of leadership experience preferred.
- Phlebotomy experience
- Outpatient and pediatric experience strongly preferred
Certificates, Licenses, Registrations:
- Current and valid license in the state of the position or compact state license
- Current CPR (BLS) certification or must be obtained within 60 days of hire
- PALS a plus

- Demonstrated clinical nursing knowledge, including assessment, evaluation, care planning, and treatment implementation.
- Effective verbal and written communication skills, including the ability to listen, educate, and influence across patient and staff interactions.
- Strong organizational skills with the ability to prioritize tasks, manage multiple responsibilities, and work independently.
- Ability to apply critical thinking and sound clinical judgment in a fast-paced environment.
- Proficiency in the use of healthcare technology, including electronic medical records and standard business software.
- Knowledge of clinic operations, including workflow coordination, supply management, and process improvement.
- Ability to support day-to-day clinic operations in collaboration with Operations Leadership.
- Strong attention to detail and commitment to accuracy in clinical and administrative responsibilities.
- Demonstrated problem-solving skills and ability to respond effectively under pressure while meeting deadlines.
- Ability to support and guide team members, fostering collaboration and a shared sense of purpose.
- Working knowledge of clinical service delivery, including quality, care delivery, and compliance considerations.
- Ability to adapt to change, demonstrate flexibility, and support continuous improvement initiatives.
- Strong patient education skills with the ability to tailor communication based on individual needs.
- Proficiency in Microsoft Office applications, including Word, Excel, and PowerPoint.
